Application Fee : $100 Please use all CAPS while typing out this application According to AL Law, “no person shall practice nor hold himself or herself out to be able to practice physical therapy or act as a physical therapist assistant in this state unless he or she is licensed in accordance with Section 34-24- 210.” Checklist 1) Request your school send an official transcript identifying degree conferred and date to the board’s office. New Graduates send a Certification of Physical Therapy Education (CPTE) until your transcripts become available. 2) Have a scanned copy of your current driver license, military ID, or another official photo ID ready to upload when prompted. 3) Contact FSBPT and arrange for a copy of your NPTE score report to be sent to our office. NOTE: APPLICANTS WHO ARE LICENSED BY THIS BOARD MUST SUCCESSFULLY COMPLETE THE NATIONAL PHYSICAL THERAPY EXAMINATION (NPTE). New Graduates: All temporary licenses are immediately voided if the applicant fails the NPTE examination.
